Notice of Termination of Prescriptions by Telephone Consultation
In order to prevent the spread of COVID-19, we have been issuing prescriptions by telephone consultations as a temporary measure, but due to a notice from the Ministry of Health, Labor and Welfare, this measure will end on Monday, July 31, 2023.
Accordingly, we will no longer issue prescriptions for telephone consultations as of the same day.
In order to get prescriptions, visiting our clinic is needed on and after August 1, 2023. We ask for your understanding and cooperation.

Please note that:
1.Patients who have been prescribed the medications at our clinic before are eligible.
2. We can only prescribe the same medications as before.
3. There is a possibility that the doctor will ask you to visit our clinic, if it's inevitable.
1.Please prepare the health insurance card and our membership card(if you have) before you call us.
We would ask;
・Patient's name/phone number
・Membership card number
・Details of the insurance card
・Name and phone and FAX number of the pharmacy
We would decide the appointment date(including today, if it's available).

2.We would call you on the appointment date.
We might not be able to call you on time if the clinic is crowded. The appointment would be cancelled if you do not answer.
3.After the consultation over the phone, we will fax the prescription to the pharmacy.
Please call the pharmacy before you go there.
4.Please complete the payment on the next visit.
Please bring the health insurance card and complete your payment on the next visit, and take the original prescription to the pharmacy after you come to our clinic.
